一、总则：

第一条为了适应我国社会主义市场经济体制下卫生事业的发展，满足特殊阶层的医疗保健需求，医院设立医疗保健金（银）卡会员门诊。

第二条医疗保健金（银）卡会员门诊的服务宗旨是充分发挥医疗技术设施完备，地理位置优越的优势，开展与之相配套的各项医疗保健服务，探索一种社区与医院相互联系为特殊阶层提供医疗保健服务的新模式。

二、会员

第三条为了便于管理和开展业务，实行会员制管理。

第四条有意成为会员的成年人（18周岁以上）可以直接书面

申请或委托他人办理入会手续。

第五条会员分为金卡会员、银卡会员。每年会员费的标准和服务内容请参阅会员协议书。

第六条会员身份的有效期为一年，期满后会员可以在办理必要手续后自由续约。

三、入会和会员卡使用

第七条入会时用户需提供的个人证件和证明包括身份证和其他有效证件，1寸个人免冠相片2张。

第八条用户在入会时必须准确填写会员登记表，并正式签署

《医疗保健金卡（银卡）会员协议书》。

第九条会员联系地址、电话如发生变更，应及时通知院方。

第十条会员卡如有遗失，会员应及时与院方联系办理原卡的注

销和新卡补办手续，并交纳补办手续工本费。

第十一条会员如对会员门诊服务持有疑义，可在加入后的一月内无条件要求退出，办理部门根据以下原则向会员退费，逾期将不再受理会员的退会事务：

一月内已享受过任何一项特需医疗服务者，仅退还该年会员费的10%以及实际结余的医疗保证金；一月内未享受过任何一项特需医疗服务者，退还实际结余的医疗保证金以及该年会费的85%，另15%会员费作为入会的手续费不予退还。

四、医疗保健金（银）卡门诊职责

第十二条医院在院区范围内和都旺社区医疗站为会员提供以下医疗服

1、按照特需医疗保健标准，为会员提供优质的服务，确保会员方便、舒适地完成各项医疗过程；

2、建立会员医疗档案，制定专人妥善管理会员病案，确保会员隐私；

3、及时提供医院的基本情况介绍，供会员咨询查阅；对会员所提出的医疗或保健问题作出合理的解答；

4、及时落实会员所提出的专家门诊、住院、手术的预约请求，保证会员得到快捷、方便的服务；

5、安排副主任医师以上的专家为会员在预约的时间段进行诊疗；

6、可根据会员的需求，为其安排特邀专家门诊、会诊及手术；

7、每一年度免费为会员进行一次体检，根据会员的自身特点制订个性化查体方案，并给出阶段性健康状况结论，以完善医疗档案；
